Medication Management— Coordinate with home health personnel so that they can assist
the client in proper dosing. Carol can also manage prescriptions, coordinating refills with
physicians and area pharmacies.

Environmental Changes — We can provide the following suite of support services:

a.  Ambulette ride along
b.  Patient comfort and reassurance during a time of potential disorientation
c.  Assessment of new environment
d.  Check in
e.  Room set-up, arranging for comforts and necessities such as TV and phone service,
     establishing relationships with key staff in order to brief them on patients’ needs, routines,
     long–term medical care and family expectations.
